For referring clinicians

Sending a referral to Greenwich Bay

A short path from your office to your patient's first visit, and back to you.

  1. 01

    Step 1: You send

Send the referral by phone or secure message to the Warwick front desk. Include your patient's name, phone number and plan, and the reason for the referral in your words.

  2. 02

    Step 2: We schedule

We contact your patient, choose the office closest to them, and tell your office the date of the first visit.

  3. 03

    Step 3: We send back

After the evaluation we send a written summary and the plan of care to your office, and an update when the plan ends or changes.

What helps us

Anything you already have. Nothing delays a first visit.

  • The referral

Your patient's name, date of birth if your system sends it, phone number and plan.

  • Your precautions

Any limits you want us to follow, such as activity or weight bearing, in your own words.

  • Who else to inform

Other clinicians you want to receive our summary.

  • Your direct line

The best number to reach your office about your patient.

Who treats your patient

Titles spelled out, so you know who will see your patient

Each program has a lead physical therapist, and several hold a board certification awarded by the American Board of Physical Therapy Specialties. A physical therapist examines every patient and writes the plan; an assistant may help under their direction.

  • Stroke program: Anastasia Kuznetsov and Faduma Osman

  • Multiple sclerosis program: Ronnie Witherspoon

  • Balance program: Manjit Jayasuriya

Questions from referring offices

Do you need a prescription or order?

In Rhode Island, you can start without a referral. Your plan may still ask for one. If your patient's plan needs an order, send it with the referral.

How will we hear about our patient?

We send a written summary after the evaluation, and an update when the plan changes or ends. Call the front desk any time.

Do you see patients on the same day for a new problem?

A sudden severe headache, new weakness or numbness, or trouble speaking is an emergency and needs 911, not a therapy visit.
